All documents must be legible

All persons over the age of 18 that will be living in the household

must be present at time of application

Preferences are;

Elderly Single- age 62 and older. No other persons in household.

Disabled Family- Head of household, spouse or Co-head is disabled- Must provide certification of the disabled status

Domestic Violence- Copy of recent Restraining Order, Police Report, and/or the Federal Certification of Domestic Violence form.

Veteran- Member of the Armed Forces who served 180 days active duty with honorable/medical discharge. Must provide copy of DD214. This applies to Public Housing only

Birth Certificate or Birth Card for every person in the household

Original Social Security Card issued by the Social Security Administration for every person in the household.

State issued Driver's License or Photo ID current and valid for every person in the household age 18 and older.

Bank Statement If any household member has a savings or checking account

Documentation of Marriage, Divorce, or Separation

Legal Custody Paperwork for non-biological dependents, foster, adopted, etc.

Expected Due Date Paperwork for pregnant applicants
